| You should file a new or amended Form W-4 if your filing status, exemption allowance, or exempt status has changed. If you claim EXEMPT from withholding tax, you must re-file by February 15th of each year if you want to continue your EXEMPT status. |

| Unless you file a State DE-4, your State withholding tax will be based on your Federal W-4 when you are first employed. Thereafter, you must file a separate certificate for each change you want to make, Federal and/or State. |

You may file new W-4's and DE-4's anytime during the year, more than once, if necessary. Although forms are available in the Payroll Office, other offices, such as Human Resources, may also have a supply. You must submit the completed form(s) to the Payroll Office as early in the pay month as possible so that your changes can be implemented. Please check your paystub (see Page 4 for an example) to verify that your new exemptions have been updated to the payroll system. |
